ELIGIBILITY QUIZ

This application form must be completed truthfully and in its entirety with all questions and documents uploaded in full, or your application will be considered incomplete and denied.

Once all documents have been gathered, the application process will take approximately 30 minutes to complete.

What documents do I need to have ready?

In order to complete this application you will need to have the following documents available electronically to upload:

  1. Recent picture of you and your family
  2. Valid form of ID*
  3. Social Security card*
  4. Two (2) most recent paystubs equal to 30 days of pay*
  5. Benefits letter(s)* (if part of your income)
  6. Two (2) most recent W2s*
  7. Two (2) most recent bank statements*
  8. Two (2) most recent completed tax returns (tax transcripts are NOT accepted)
    * Required for each applicant
